At the end of a tutoring session, we ask that tutors fill out a client report (also known as a post-session report) that will capture valuable information about the appointment and inform Academic Commons Directors on how to best improve tutoring services for the end-user.
To create a Client Report, you may either click on the My Appointment link at the top of the page or appointment areas on either the calendar or client display.
If clicking the Open Appointment link, staff can also access this feature by clicking the blue Appointment Tools link on the resulting page.
Client Reports can be opened or edited at any time.
Staff are requested to complete the form in its entirety, though they may use 'NA' for answers that are not applicable or unknown. Shared assignments or additional documents can also be added to the form.
